ABSTRACT

Objective:To analyze the knowledge, skills and self-efficacy of cardiopulmonary resuscitation among the public in mountainous areas of southern Ningxia, and provide scientific support for the popularization of cardiopulmonary resuscitation.Methods:According to inclusion and exclusion criteria, a total of 1 503 inhabitants in Ningxia district (Jan. 2018 to Jan. 2019) were chosen as research samples. The general data was analyzed; the self-made questionnaire was applied to evaluate the inhabitants ′ theory knowledge, skill mastery and self-efficacy by professional nurses. Multiple linear regression was used to analyze the influence factors of theory knowledge and skill mastery degree. Results:A total of 1 503 valid questionnaires were collected, the assessment scores of theory knowledge, skill mastery and overall self-efficacy were (53.11±12.34), (62.64±18.35) and (6.09±2.18) points; According to demographic characteristics, multiple linear regression indicated that the influence factors of theory knowledge and skill mastery assessment scores included age, occupational distribution, education backgrounds, training conditions, time of taking training to survey, "onlooker" experience and self-efficacy indicators ( t/ F values were 5.723, 4.393, 5.817, 6.435, 5.990, 6.438, 6.631, P<0.01). The positive correlations were assumed ( r values were 0.012, 0.016, P<0.05) . Conclusion:In Ningxia, the inhabitants generally have low theory knowledge, skill mastery and self-efficacy levels. Therefore, the relevant medical institutions shall enhance the inhabitants ′ cardio-pulmonary resuscitation training through multiple methods and approaches.

ABSTRACT

Objective:To analysis the clinical characteristics of The clinical characteristics of using automated external defibrillation in the public place,To explore the feasibility and effectiveness of AED application in public places in China.Methods:From January 2014 to April 5, 2019, 54 cases of on-site emergency medical records of AED use in public places in China were analyzed retrospectively from three aspects: patient and AED user attributes, and AED clinical performance.Results:After field application of AED analysis, 54 patients did not have out of hospital cardiac arrest in 9 patients; cardiac arrest in 45 patients, cerebral resuscitation in 40 patients (88.9%), death in 5 patients (11.1%), one of them died in hospital. The accuracy of AED for defibrillation rhythm recognition and defibrillation recommendations was 100%. The success rate of shock to VF was 97.22%, and that of non pulse VT was 100%. The data shows that AEDs of different brands show clinical effectiveness in the core indicators of work. The operation level of the rescuer determines the critical time of AED shock, which is closely related to the prognosis of the patient ( P<0.05) . Conclusions:AED is reliable and effective in electric shock decision and performance.The overall efficiency of AED application can be improved by strengthening training, shortening the critical time of electric shock, rational configuration and effective management.

ABSTRACT

Objective:To study clinical characteristics and risk factors of young patients with coronary heart disease (CHD) complicated metabolic syndrome (MS).Methods: A total of 200 CHD young patients were selected from our hospital, including 100 patients with MS (CHD+MS group) and 100 patients without MS (CHD control group).Lesion severity was evaluated according to coronary lesion score.General data were compared between two groups,and risk factors were assessed CHD+MS group.Results: Compared with CHD control group, there were significant rise in body mass index (BMI), levels of total cholesterol (TC), 2h postprandial blood glucose (2hPG), fasting blood glucose (FBG), triglyceride (TG), blood pressure, serum uric acid (UA) and creatinine (Cr) and homeostasis model-insulin resistance index (HOMA-IR), and significant reduction in level of high density lipoprotein cholesterol (HDL-C) in CHD+MS group, P<0.01 all.Compared with CHD control group, there were significant rise in incidence rate of multi-vessel coronary disease (6% vs.32%) and Gensini score of coronary angiography [(5.1±2.0) scores vs.(8.4±5.3) scores] in CHD+MS group, P<0.01 both.Logistic regression analysis indicated that low HDL-C, HOMA-IR, hypertension and diabetes mellitus history were risk factors for CHD complicated MS in young patients(OR=1.097~2.246,P<0.01 all).Conclusion:There are much risk factors and abnormal metabolism,and coronary disease are more serious in young patients with CHD complicated MS.

ABSTRACT

Objective To explore the diagnostic value of combined bedside detection of aminoterminal pro-B-type natriuretic peptide (NT-proBNP) and cardiac troponin I (cTnI) in the diagnosis of cardiac dyspnea. Methods A total of 120 patients with dyspnea admitted in our department from June 2014 to February 2016 were included in this study. At the time of admission, NT-proBNP and cTnI levels were measured by bedside test. Values of NT-proBNP>300 ng/L or cTnI>0.16 mg/L were defined as positive for cardiac dyspnea. According to the final diagnosis, patients were divided into two groups:cardiac dyspnea group (n=68) and pulmonary dyspnea group (n=52). At the same time, 30 healthy people were selected as control group. Values of NT-proBNP and cTnI were used for statistical analysis between the three groups. The sensitivity and specificity of NT-proBNP, cTnI and cTnI+NT-proBNP were compared between three groups. Results The levels of NT-proBNP and cTnI were significantly higher in pulmonary dyspnea group and cardiac dyspnea group than those in the control group, and the levels were significantly higher in cardiac dyspnea group than those of pulmonary dyspnea group (P<0.01). The detection sensitivity of NT-proBNP and cTnI alone was 67.65% and 52.94%, combined detection of both was up to 94.12%. The specificity of NT-proBNP and cTnI detection alone was 70.00% and 53.33%, respectively, and combined detection of both was up to 86.67%. The sensitivity of NT-proBNP+cTnI was significantly higher than that of NT-proBNP and cTnI alone (P<0.05), but there was no significant difference in the specificity between combined detection andindividual detection of NT-proBNP. The positive predictive value of the combined detection in the diagnosis of cardiac dyspnea was 94.12%(64/68), and the negative predictive value was 86.67%(26/30). Conclusion Bedside detection with combination of cTnI and NT-proBNP has important clinical application value in the rapid diagnosis of cardiac dyspnea, which is a rapid clinical testing method.

ABSTRACT

The pre-treatment serum levels of carbohydrate antigen 125 (CA125),cytokeratin 19 fragment 21-1 (CYFRA21-1),squamous carcinoma associated antigen (SCC-Ag),and tissue polypeptide antigen (TPA) were measured in 111 patients with primary non-small-cell lung cancer.And 103 benign lung disease cases and 97 healthy volunteers were also studied as controls.Enzyme linked immunosorbent assay (ELISA) was employed to detect the serum contents of SCC-Ag and TPA.The levels of CA125 and CYFRA21-1 were measured by electrochemical luminescence.The serum level of CYFRA21-1,SCC,TPA and CA125 in cancer patients were significantly higher than those of normal control and benign disease groups (P < 0.01).The combined detection of tumor markers could significantly improve the sensitivity and specificity.And the levels of tumor markers were also correlated with the clinical stages and differentiation degrees.

ABSTRACT

Objective To investigate serum levels of sialic acids(SA)vs carcino-embryonic antigen(CEA)for the diagnosis of digestive tract cancer.Methods Sixty healthy adults,100 patients with malignant digestive tract tumor and those undergoing radical digestive tract tumor surgery were enrolled in this case-control study.Serum SA level was tested.Serum CEA level was measured by electrochemical luminescence.Sensitivity and specificity of SAvs CEA were calculated.Receiver operating characteristic (ROC)curve and area under the curve(ROC-AUC)were used to evaluate the diagnostic value of SA.Results Serum level of SA of patients with digestive malignant tumors was much higher than the normal controls(P <0.01),although serum SA of patients undergoing tumor resection showed no difference with the normal controls(P > 0.05).There was significant difference in SA level between the patients who received surgical therapy or not.The sensitivity and specificity of SA in diagnosing digestive tract cancer was 55.00% and 93.3%,respectively.The false positive rate was 6.7% in normal subjects.Conclusion Serum SA or CEA testing may be equally useful in the screening,diagnosis,outcome assessment and followup study of digestive tract cancer.

ABSTRACT

OBJECTIVE To provide laboratory evidence for the prevention and control of coagulase-negative staphylococci(CNS) and study the prevalence of meticillin resistance and vancomycin resistance in clinical CNS in our hospital.METHODS Meticillin-resistant coagulase-negative staphylococci(MRCNS) were detected with cefoxitin disk diffusion and mecA-PCR.CNS with reduced susceptibilities to vancomycin was detected with vancomycin agar screen test.Their MIC was determined with E-test and gene van was detected with multiplex PCR.RESULTS CNS was highly resistant to cephem,?-lactam,aminoglycoside,macrolide,and lincosamide.The confirm rate of cefoxitin disk diffusion to mecA-PCR was 96.2% and 92.3% of mecA in 52 CNS strains were detected.Four strains of staphylococci with heteroresistance to vancomycin(VRS) were screened in 256 CNS ones which were all MRCNS but without gene van detected.CONCLUSIONS It is important for clinical laboratory to detect VRS and MRCNS with suitable methods to prevent VRS infection and prevalence.

ABSTRACT

The renal tubular markers are very important for early diagnosis of renal tubulointerstitial injuries. Urinary excretion of Cystatin C is a sensitive marker of early injury in renal proximal tubule. The particle turbidimetric immunoassay developed in recent years has been used in the autoanalyzer, and may be used in medical laboratories routinely. Urinary cystatin C excretion has early diagnostic value for diabetic nephropathy, hypertensive nephropathy, and drug-induced renal impairments. This tubular marker will be used more frequently for the diagnostic purpose, as the trend of various chronic kidney diseases will be increased in the near future. The current problems to be solved are the standardization of methodology, and the further study according to the guidelines of evidence-based laboratory medicine. In the routine testing, the ratio of a spot urine Cystatin C concentration to creatinine is recommended.

ABSTRACT

AIM: To investigate the expression of CD40 and CD40 ligand(CD40L) during myocardial ischemia/reperfusion injury in rats. METHODS: In the rat modal with myocardial ischemia/reperfusion(MI/R),the changes of CD40 and CD40L expression in blood and myocardial tissues were detected by flow cytometry and immunohistochemistry, respectively. There were seven animal groups in the study, including the normal group ( n= 3), the ischemia 30 min group(I 30 min , n= 6),the ischemia/reperfusion 1 min,5 min,10 min,20 min and 30 min groups(each group, n= 6).
